Dentistry is not general practice with different instruments. The visit is shaped differently, the record is shaped differently, and the money is shaped differently.
What we decided early
Three decisions shaped everything that followed.
A record is never overwritten. A correction is a new version of the entry, and the old one stays readable. A clinical record whose history can be edited is not a record.
Radiographs stay in your imaging system. We hold a link and never a copy. It is the single largest category of sensitive data in a dental practice, and the safest way to hold it is not to.
Access is denied by default. Every endpoint refuses until a role is granted it, and a clinic can narrow what a role may do but never widen it, so "receptionist" means the same thing in an audit trail read at a different practice.
